Your menstrual cycle has 2 phases:

The follicular phase –

The follicular phase is the longest step in the menstrual cycle, lasting from the first day of a period to ovulation, meaning the release of the egg. This critical step in the development of an egg pre-fertilization (meaning penetrative sex), and can last between 11 and 27 days.

The luteal phase –

The luteal phase typically ranges from 12-14 days and produces Progesterone. These levels peak 6-8 days after ovulation. Progesterone is secreted in response to LH (Luteinizing hormone). Progesterone is needed to support and maintain an embryo in the womb.

 

So what is Luteal Phase deficiency?

Luteal phase deficiency (LPD) is when this phase is less than 10 days for any given patient. This may indicate that there is either insufficient hormone production to maintain an endometrium OR that there are enough hormones, but the endometrium is not responding to it.

In some cases, LPD can be due to pathological conditions such as polycystic ovarian syndrome, thyroid dysfunction, eating disorders, and hyperprolactinemia. When we are planning for an embryo transfer, we will rule out these conditions and treat the underlying cause.

Why diagnosing LPD is important:

The luteal phase is an important part of the menstrual cycle during which implantation can occur and pregnancy can be established. Abnormalities in the luteal phase could affect implantation and pregnancy.

Treating LPD:

If there is a concern of LPD, you will be started on medications such as Prometrium to ensure that the embryo will be supported. Progesterone injections, pills, or suppositories may be used after ovulation to help the lining of your uterus grow. The ideal uterine lining thickness for transfer is 7mm.
