If you feel comfortable and have decided to apply to become a donor, please download and fill out the Donor Application and Health and History Form. Please also complete the W-9 form to request for your SSN. Donor compensation may be taxable and you will receive a 1099 form at the end of the year.
If you would like to have the forms emailed, please contact us at donors@nhfc.com. Along with your completed application form, please refer to the photo submission instructions. Incomplete application forms will not be processed.
